These Terms of Service (“Terms”) govern your use of CS Logbook, operated by Gleason Digital LLC (“we,” “us,” or “our”), a Nevada limited liability company. By accessing or using CS Logbook Sheets or CS Logbook Online, you agree to these Terms. If you do not agree, do not use the Service.
CS Logbook currently offers two products:
We grant you a limited, non-exclusive, non-transferable license to use CS Logbook for your internal veterinary practice operations. You may not sublicense, resell, or distribute CS Logbook to third parties.
For CS Logbook Sheets, your license permits use at the number of clinic locations specified in your subscription plan. You may not share your license across organizations.
You must provide accurate registration information. You are responsible for maintaining the confidentiality of your account credentials. Notify us immediately at support@cslogbook.com if you suspect unauthorized use of your account.
You must be at least 18 years old and authorized to enter into contracts on behalf of your organization to create an account.
CS Logbook Sheets is billed annually. Fees are charged at the beginning of each billing period. Subscriptions auto-renew unless cancelled at least 24 hours before the renewal date.
All fees are non-refundable except as required by applicable law or as otherwise stated in a separate written agreement. We reserve the right to change pricing with 30 days' notice.
CS Logbook is designed to assist veterinary professionals with DEA controlled substance record-keeping under 21 CFR Part 1304 and applicable state regulations. However:

CS Logbook and its original content, features, and functionality are owned by Gleason Digital LLC and are protected by applicable intellectual property laws. Your controlled substance records belong to you.
You may cancel your subscription at any time through your account settings. We may suspend or terminate your access if you violate these Terms, with or without notice.
Upon termination of a CS Logbook Online account, we will provide a 30-day window to export your records before deletion. CS Logbook Sheets data is stored in your Google Drive and is unaffected by termination.
TO THE MAXIMUM EXTENT PERMITTED BY NEVADA LAW, GLEASON DIGITAL LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, INCLUDING LOST PROFITS, REGULATORY FINES OR PENALTIES, OR DATA LOSS, ARISING FROM YOUR USE OF CS LOGBOOK.
OUR TOTAL LIABILITY TO YOU FOR ANY CLAIMS ARISING UNDER THESE TERMS SHALL NOT EXCEED THE FEES YOU PAID TO US IN THE 12 MONTHS PRECEDING THE CLAIM.
CS LOGBOOK IS PROVIDED “AS IS” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF SECURITY VULNERABILITIES.
These Terms are governed by the laws of the State of Nevada, without regard to conflict of law principles. Any disputes shall be resolved in the state or federal courts located in Douglas County, Nevada, and you consent to personal jurisdiction in those courts.
We may update these Terms from time to time. We will notify you by email and by posting a notice on our website at least 14 days before material changes take effect. Continued use of CS Logbook after the effective date constitutes acceptance of the updated Terms.
