These Terms of Service (“Terms”) govern your use of CS Logbook, operated by Gleason Digital LLC (“we,” “us,” or “our”), a Nevada limited liability company. By accessing or using CS Logbook, you agree to these Terms. If you do not agree, do not use the Service.
CS Logbook Online is a cloud-hosted web application for veterinary controlled substance record-keeping. Your controlled substance records are stored on CS Logbook infrastructure hosted in the United States.
We grant you a limited, non-exclusive, non-transferable license to use CS Logbook for your internal veterinary practice operations. You may not sublicense, resell, or distribute CS Logbook to third parties.
Your subscription is priced per DEA-registered veterinarian: one seat covers one DEA registrant. Clinics and staff members are unlimited and are not billed separately. A practice with multiple registered veterinarians pays for one seat per registrant. You may not share a seat across registrants or organizations.
You must provide accurate registration information. You are responsible for maintaining the confidentiality of your account credentials. Notify us immediately at support@cslogbook.com if you suspect unauthorized use of your account.
You must be at least 18 years old and authorized to enter into contracts on behalf of your organization to create an account.
CS Logbook is billed per DEA-registered veterinarian on a monthly or annual basis, as selected at checkout. Fees are charged in advance at the beginning of each billing period. Subscriptions auto-renew unless cancelled at least 24 hours before the renewal date.
All fees are non-refundable — this applies to both monthly and annual plans — except as required by applicable law or as otherwise stated in a separate written agreement. Your subscription is cancelable only with effect at the end of your then-current billing period; we do not provide refunds or prorated credits for cancellation, or for reducing the number of veterinarians on your plan, during a billing period. Any reduction takes effect at the next billing period. We reserve the right to change pricing with 30 days' notice.
CS Logbook is designed to assist veterinary professionals with DEA controlled substance record-keeping under 21 CFR Part 1304 and applicable state regulations. However:
Use of CS Logbook is in no way a guarantee of compliance with DEA, state veterinary board, state prescription-monitoring, or any other regulatory requirements. The DEA registration and all associated record-keeping obligations belong to the individual veterinarian registrant and the practice, not to CS Logbook or Gleason Digital LLC. You are solely responsible and liable for your regulatory compliance and for any action taken or penalty assessed by any governmental authority in connection with your records or practice.

CS Logbook and its original content, features, and functionality are owned by Gleason Digital LLC and are protected by applicable intellectual property laws. Your controlled substance records belong to you.
You may cancel your subscription at any time through your account settings; cancellation takes effect at the end of your then-current billing period, and no prorated refund is provided for the remainder of that period (see Section 4). We may suspend or terminate your access if you violate these Terms, with or without notice.
Upon cancellation, your account enters a read-only grace period during which you may export your records. Because controlled substance records are subject to DEA and state retention requirements, they are preserved for the mandatory retention period and are not deleted during that period; we do not destroy your regulated records on demand while a retention obligation applies.
TO THE MAXIMUM EXTENT PERMITTED BY NEVADA LAW, GLEASON DIGITAL LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, INCLUDING LOST PROFITS, REGULATORY FINES OR PENALTIES, OR DATA LOSS, ARISING FROM YOUR USE OF CS LOGBOOK.
OUR TOTAL LIABILITY TO YOU FOR ANY CLAIMS ARISING UNDER THESE TERMS SHALL NOT EXCEED THE FEES YOU PAID TO US IN THE 12 MONTHS PRECEDING THE CLAIM.
CS LOGBOOK IS PROVIDED “AS IS” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF SECURITY VULNERABILITIES.
You are responsible for verifying the accuracy of your records and for maintaining your own independent backups of any data that is important to you. You should not rely on CS Logbook as your sole copy of any record you are legally required to retain.
These Terms are governed by the laws of the State of Nevada, without regard to conflict of law principles. Any disputes shall be resolved in the state or federal courts located in Douglas County, Nevada, and you consent to personal jurisdiction in those courts.
We may update these Terms from time to time. We will notify you by email and by posting a notice on our website at least 14 days before material changes take effect. Continued use of CS Logbook after the effective date constitutes acceptance of the updated Terms.
You agree to defend, indemnify, and hold harmless Gleason Digital LLC and its officers, members, employees, and agents from and against any claims, liabilities, damages, judgments, losses, costs, or expenses (including reasonable attorneys' fees) arising out of or relating to your controlled-substance activities, your records and the data you enter, the acts or omissions of your authorized users, your use or misuse of CS Logbook, your violation of these Terms, or your violation of any law or regulation, including any DEA, state veterinary board, or state prescription-monitoring requirement applicable to your practice.
You have no obligation to indemnify us to the extent a claim is caused by Gleason Digital LLC's gross negligence, willful misconduct, fraud, violation of law, breach of its security obligations, or a defect in CS Logbook that is directly caused by us and not attributable to your misuse, configuration, or data entry.
Any cause of action or claim you may have arising out of or relating to these Terms or your use of CS Logbook must be commenced within one (1) year after the cause of action accrues; otherwise, such cause of action or claim is permanently barred, to the maximum extent permitted by applicable law.
CS Logbook is operated from the United States and is intended for use only by veterinary practices and professionals located in the United States and its territories. We make no representation that CS Logbook is appropriate or available for use outside the United States. If you access CS Logbook from elsewhere, you do so on your own initiative and are responsible for compliance with local laws.
Severability. If any provision of these Terms is held invalid or unenforceable, that provision will be limited or eliminated to the minimum extent necessary, and the remaining provisions will continue in full force and effect.
Waiver. Our failure to enforce any right or provision of these Terms will not be deemed a waiver of that right or provision.
Entire Agreement. These Terms, together with our Privacy Policy and any order or written agreement you enter into with us, constitute the entire agreement between you and Gleason Digital LLC regarding CS Logbook and supersede all prior understandings.
Additional terms apply to your use of CS Logbook Online and are incorporated into these Terms by reference: our Data Processing Addendum, our Subprocessor list, and — for DEA registrants — the Registrant Responsibility Acknowledgment. These documents are available within your account and form part of these Terms. Where an additional term conflicts with these Terms, these Terms control unless the additional document expressly states otherwise.
