DEA Compliance → Theft & Loss (DEA Form 106)
Purpose
Record a theft or significant loss of controlled substances in CS Logbook — the incident, the affected substances, and your investigation — so you keep a complete internal record and a reminder that the DEA's reporting channel still needs to be used. The official notification and DEA Form 106 are filed with the DEA, not through CS Logbook.
This is general product information about CS Logbook, not legal advice. CS Logbook does not notify the DEA or file Form 106 for you, and does not define the reporting timeline. Verify the current requirement on the DEA Theft/Loss Reporting page.
In CS Logbook
The theft/loss workflow lets you:
- Create an incident, attach the affected substances, and track an investigation (follow-ups, witnesses) through to resolution.
- See a soft reminder when an open incident is approaching the DEA's stated notification window, so the external filing does not slip. The reminder is informational — it never blocks you, and the authoritative timeline is the DEA's, not CS Logbook's.
How to record and work an incident:
CS Logbook records the incident and your investigation; you still notify the DEA and submit Form 106 through the official channel below.
Where the official requirement lives
CS Logbook does not set the reporting timeline or receive the filing. The authoritative sources are:
- DEA Theft/Loss Reporting — notification expectations and how to file Form 106.
- DEA Theft/Loss Reporting Online (TLR) — the online filing system.
- DEA Theft or Loss Q&A.
- eCFR — Title 21, Part 1301 (Registration) — the theft/loss reporting regulations.
Follow the DEA's stated timeline and process — not this guide — for when and how to report.
